A Straightforward Approach to Accreditation
HFAP is a nationally recognized healthcare facility accreditation
organization, with deeming authority from the Centers for Medicare
and Medicaid Services (CMS). HFAP meets or exceeds the standards required by
CMS to provide accreditation to all hospitals, ambulatory care/surgical
facilities, mental health facilities, physical rehabilitation facilities, clinical
laboratories, and critical access. HFAP also provides certification to primary stroke centers.
HFAP also is recognized by:
- National Committee for Quality Assurance (NCQA)
- Accreditation Council for Graduate Medical Education (ACGME)
- State Departments of Public Health
- Managed care organizations
- Insurance companies
Clear and Direct Survey Standards
The basis of the HFAP standards are the Medicare Conditions of Participation
(CoPs) for each type of facility. The standards for each program are cross-walked to the CoPs. This cross-walk approach means anyone reading
the HFAP standards manual can clearly see how each standard ties directly to
the Medicare requirements.
HFAP standards are composed of all Medicare standards plus standards proven to elevate
quality and enhance patient safety.
Compliance with HFAP requirements assures compliance with Medicare standards. Approximately 80% of HFAP standards are corss-walked to the Medicare CoPs and associated requirements.

High-Quality, Helpful Surveyors
Our surveyors are experienced medical professionals, most still in active practice, with practical field experience
and a sincere, helpful attitude. When a deficiency is identified, they are ready
to discuss options in real time.
HFAP accreditation survey teams generally consist of three surveyors:
A physician who serves as team captain, along with a registered nurse and hospital administrator.
To ensure surveyors maintain a high level of professional quality, HFAP requires:
- All surveyors participate in mandatory training workshops to augment the
real-world experience they bring with them
- HFAP surveyor teams are not "fixed", so surveyors work with many other surveyors
Comprehensive, Straight-Forward Survey Process
The HFAP facility accreditation process consists of only 5 basic steps, from application
to accreditation. The size of the facility being surveyed and the results of the
actual survey determine how long the process takes and are the only reasons the
process may vary.
The basic steps in the accreditation surveying process include:
- Application
- Survey
- Deficiency report
- Plan of corrections/Correct action response
- Accreditation action
When switching your accreditation to HFAP our staff will work with you to ensure
there is no interruption in accreditation and reimbursement. Ideally, we would like to
begin the application process at least six months prior to the expiration date
of your current accreditation.
Cost-Effective Quality Survey

Determining the HFAP facility accreditation survey fee is straight-forward,
varying only by the size and volume of your organization. And because our standards
are so clearly written, additional consultations and workshops are not typically required. We also provide our Accreditation Manual electronically with no licensing restrictions at no cost. The electronically manual is highly interactive and allows your organization to use it, manage it, reproduce it and employ it as best fits your compliance strategy.
There are three main elements to the HFAP fee for accrediting a
facility:
- Triennial Registration Fee
- Access to the On-Line Standards Manual (no cost)
- Reimbursement for direct costs of conducting the survey, depending
on the number of surveyors assigned to your facility and their travel costs.
