            <title>Impacting Patient Outcomes Through an Improved Discharge Process</title>
            <description>On 6/10/2010, the Healthcare Facilities Accreditation Program (HFAP) and the Bureau of Hospitals (BOH) held a collaborative webcast on improving the hospital discharge process through the use of key HFAP standards. .  The featured speaker for the webcast was Jerome A. Dixon, DO, a board-certified family physician and OMT specialist who practices medicine in Campbellsville, KY.  Moderating the webcast was BOH Chair Paul E.             <title>HFAP - the first accrediting organization to join the Patient-Centered Primary Care Collaborative Executive Committee</title>

                <![CDATA[The Healthcare Facilities Accreditation Program (HFAP) has become the first accrediting organization to join the Patient-Centered Primary Care Collaborative (PCPCC) Executive Committee.<br />
<br />
The PCPCC was created in late 2006 when approached by several large national employers with the objective of reaching out to the American College of Physicians, the American Osteopathic Association, the American Academy of Family Physicians, and other physician groups in order to facilitate improvements in patient-physician relations and create a more effective and efficient model of healthcare delivery. <br />

            <title>Standard 10.01.03 is Retired for Acute Hospitals &amp; Critical Access Hospitals</title>
            <description>Standard 10.01.03 is being retired, effective immediately for Acute Hospitals and Critical Access Hospitals.  At it&apos;s May, 2010 meeting, the Bureau voted to retire the standard due to conflicting language with standard 25.02.03.  All hospital and critical access hospital customers are requested to remove this standard from their standards manual.</description>

            <title>OIG Report Evaluates Adverse Event Reporting Methods in Hospitals</title>
            <description>A report by the Department of Health and Human Services Office of Inspector General (HHS OIG) finds that reviewing medical records made by nurses and physicians may be the most effective way to monitor the occurrence of adverse events in hospitals. The report, titled Adverse Events in Hospitals: Methods for Identifying Events, implemented five screening methods to a random sample of 278 Medicare beneficiary hospitals over a 2-week period in 2008. The case study was performed in accordance with the Tax Relief and Health Care Act of 2006, which requires OIG to publish regular reports on efforts to better identify adverse medical events.  OIG has recommended that the Centers for Medicare &amp; Medicaid Services (CMS) and the Agency for Healthcare Research and Quality (AHRQ) continue to research methods for identifying adverse events in the review of medical records.
